Receptor profile table comparing semaglutide, tirzepatide and retatrutide at the GLP-1, GIP and glucagon receptors, with fatty-acid side chain, half-life and US status

Retatrutide vs Tirzepatide vs Semaglutide: A Research Comparison

For laboratory research only. Not for human or animal use. This article gives no dosing, administration or therapeutic advice.

Short answer: semaglutide, tirzepatide and retatrutide are three long-acting peptides that differ mainly in how many hormone receptors they activate. Semaglutide is a GLP-1 receptor agonist,[1] tirzepatide acts on the GLP-1 and GIP receptors,[2][3] and retatrutide acts on the GLP-1, GIP and glucagon receptors.[4] Their regulatory positions differ just as much: semaglutide and tirzepatide are FDA-approved prescription drugs, while retatrutide remains an investigational drug in phase 3 trials.[5][6] This article compares their design and the published study record. It makes no efficacy or treatment claims and gives no doses.

One receptor, two receptors, three receptors

All three molecules start from the incretin idea: gut hormones such as GLP-1 and GIP act on G protein-coupled receptors that regulate insulin secretion and appetite signaling. Native GLP-1 is cleared within minutes, so each drug program engineered its peptide for stability.

Semaglutide keeps the GLP-1 backbone with two substitutions (Aib at position 8, Arg at 34) and a fatty-acid derivative on lysine 26. In its discovery paper, the albumin affinity rose while GLP-1 receptor affinity fell about three-fold compared with liraglutide, and plasma half-life in mini-pigs was 46.1 hours.[1] Cryo-EM structures show semaglutide engaging the GLP-1 receptor much like native GLP-1, with differences in receptor motion.[7]

Tirzepatide is a fatty-acid-modified peptide built to activate both incretin receptors.[2] In cell assays it matched native GIP at the GIP receptor but was biased at the GLP-1 receptor, favoring cAMP over beta-arrestin recruitment, which its authors call “imbalanced” toward GIP.[3] Cryo-EM work attributed this profile to both the amino acid sequence and the fatty-acid modification.[8] In obese mice, GIP receptor agonism contributed to insulin sensitization independent of weight change.[9]

Retatrutide (LY3437943) adds the glucagon receptor. In vitro, its activity was balanced at the glucagon and GLP-1 receptors with more activity at the GIP receptor; in obese mice, the glucagon-receptor component was linked to higher energy expenditure on top of reduced food intake.[4]

The study record, described neutrally

The table lists design, population and primary endpoint for representative studies. “Reported” restates the authors’ conclusion on the primary endpoint without effect sizes; the PMID links lead to the full abstracts.

Study Design / model Population Primary endpoint Reported Ref
Retatrutide discovery Cell assays, obese mice, phase 1 Mice; phase 1 participants Receptor activity; safety Triple agonism confirmed; profile similar to other incretins [4]
Phase 1b Randomized, placebo-controlled, multiple ascending dose Type 2 diabetes Safety and pharmacokinetics Acceptable safety; half-life about 6 days [12]
Retatrutide phase 2 Randomized, placebo-controlled, 48 weeks Adults with obesity Body-weight change Met; dose-related GI events and heart-rate increases [16]
Retatrutide phase 2 (T2D) Placebo- and dulaglutide-controlled Type 2 diabetes, USA HbA1c change Met [17]
TRIUMPH-1 Phase 3, randomized, 80 weeks Obesity without diabetes Body-weight change; knee pain and sleep-apnea subgroups Met; GI events most common [5]
TRIUMPH-2 Phase 3, placebo-controlled Obesity with type 2 diabetes Body-weight change Met [6]
SURPASS-2 Phase 3, open-label, active comparator Type 2 diabetes HbA1c, tirzepatide vs semaglutide Noninferior and superior; more serious AEs with tirzepatide [18]
SURMOUNT-1 Phase 3, placebo-controlled, 72 weeks Obesity without diabetes Body-weight change Met; GI events most common [19]
STEP 1 Phase 3, placebo-controlled Overweight or obesity Body-weight change Met; more GI-related discontinuations [20]
SUSTAIN-6 Cardiovascular outcomes trial Type 2 diabetes, high CV risk Major adverse CV events Noninferiority confirmed; more retinopathy complications [21]
evoke / evoke+ Phase 3, oral semaglutide Early Alzheimer’s disease Clinical progression Not met [22]
Three-drug kidney study Mouse fibrosis and aging models Mice Renal fibrosis markers Drug- and model-specific pathway effects [23]

Two patterns stand out. Gastrointestinal adverse events dominate the safety sections of all three programs, and retatrutide’s phase 2 trial reported dose-dependent heart-rate increases that peaked at 24 weeks.[16] The evoke program is a reminder that these drugs do not succeed in every indication tested.[22]

What the evidence does not show

  • No human head-to-head trial of retatrutide. Retatrutide has not been compared directly with tirzepatide or semaglutide in a published human trial; only tirzepatide vs semaglutide has phase 3 head-to-head data.[18]
  • More receptors is not automatically better. Each added receptor brings its own biology, including glucagon-receptor effects on energy expenditure in mice and heart-rate changes in people.[4][16]
  • Mouse comparisons do not rank drugs for people. The three-drug kidney study generated hypotheses at the doses tested in mice, by its own description.[23]
  • Trial results describe trial products. Phase 3 data apply to the sponsor’s formulated drug under trial conditions, not to research reagents.
  • Long-term data for retatrutide are still accumulating. Its phase 3 program reported only in 2026.[5][15]

Purity catalog records and how to read their COAs

Purity lists retatrutide as 10mg, 15mg, 20mg and 30mg vials plus a pen, and tirzepatide (Tirz-Luxe) as 10mg, 15mg, 20mg and 30 mg vials plus a pen. Purity sells no semaglutide. As read from the posted PDFs on October 1, 2026:

When you read one of these reports, check four things. The lot on the report should match the lot you receive; the retatrutide 10mg page links two lots, so the match matters. The compound line should name the right molecule: retatrutide and tirzepatide differ by about 82 g/mol, which mass spectrometry resolves easily. The measured quantity is reported against the label spec, so a 15mg result tells you nothing about a 20mg vial. And the endotoxin report is a separate document with its own lot suffix. Frequently asked questions

Is retatrutide FDA-approved?

No. As of October 1, 2026, retatrutide is an investigational drug in phase 3 clinical trials and has no FDA approval for any use. Semaglutide and tirzepatide are FDA-approved prescription drugs sold under brand names such as Ozempic, Wegovy, Mounjaro and Zepbound.

What is the main structural difference between the three peptides?

Receptor reach. Semaglutide is a GLP-1 analog that acts on the GLP-1 receptor. Tirzepatide adds activity at the GIP receptor. Retatrutide adds a third target, the glucagon receptor. All three carry a fatty-acid side chain that binds albumin and extends half-life.

Have the three been compared head to head?

Partly. A phase 3 trial compared tirzepatide with semaglutide in type 2 diabetes (SURPASS-2). We found no published head-to-head human trial of retatrutide against either drug; a 2026 mouse study compared all three in kidney fibrosis models.

Does Purity sell semaglutide?

No. Purity Collective lists retatrutide and tirzepatide research materials only. Semaglutide is described in this article from the published literature for comparison.

Is a research-grade tirzepatide vial the same as Mounjaro or Zepbound?

No. Mounjaro and Zepbound are FDA-approved drug products with a specific manufacturer, formulation and label. A research-grade vial is a laboratory reagent sold for in vitro and preclinical research only, and its quality is documented by its own batch COA.

Which Purity retatrutide and tirzepatide listings have a batch COA?

As of October 1, 2026, posted Horizon Analytical reports cover retatrutide 10mg (also linked from the pen), 15mg and 30mg, and Tirz-Luxe 10mg (also linked from the tirzepatide pen) and 30mg. A batch COA for retatrutide 20mg, Tirz-Luxe 15mg or Tirz-Luxe 20mg isn’t posted yet.
