What tesamorelin is

Tesamorelin (development code TH9507) is a synthetic analogue of human growth hormone-releasing hormone. It keeps the full 44-residue GHRH sequence and adds a trans-3-hexenoyl group to the N-terminal tyrosine, a small change that protects the peptide from dipeptidyl peptidase-IV and slows its breakdown in plasma^[1]. In the body it acts on the pituitary GHRH receptor; in laboratory work it is used as a stabilized GHRH reference compound. Identity

What the research says

Tesamorelin has an unusually deep published record for a research peptide because it went through a full drug-development program. The entries below name the model for each result and include the trials that did not show a benefit.

• Preclinical pharmacology (rat, dog, pig). The hexenoyl modification slowed degradation of the peptide in rat, dog and human plasma in vitro and prolonged its elimination in vivo. Repeated administration raised growth hormone and IGF-1 in pigs, rats and dogs. In dogs, four-month toxicity studies produced reversible liver, kidney, blood-count and organ-weight findings that the authors attributed to prolonged supraphysiological GH/IGF-1 exposure^[1].
• Pharmacokinetics (human). A population analysis in 38 HIV-infected and healthy adults described tesamorelin with a one-compartment model, high clearance and a short half-life; age, body size, race and health status did not change the parameters within the range studied^[2].
• Visceral fat (randomized human trials). In a 26-week placebo-controlled trial of 412 HIV patients with abdominal fat accumulation, tesamorelin reduced visceral adipose tissue on CT and improved triglycerides and the cholesterol ratio relative to placebo, while IGF-1 rose^[4]. A later 6-month trial in 50 patients found reductions in both visceral fat and liver fat versus placebo^[5].
• Liver fat (randomized human trial). In a 12-month double-blind trial of 61 people with HIV and non-alcoholic fatty liver disease, hepatic fat fraction fell with tesamorelin compared with placebo, but fasting glucose and HbA1c did not differ between groups, and injection-site complaints were more common in the treated group^[6].
• Cognition (two trials, mixed results). A 20-week placebo-controlled study of 152 older adults with and without mild cognitive impairment reported a favorable effect of GHRH analogue treatment on an executive-function composite, with IGF-1 kept in the physiological range^[7]. A 2025 phase 2 open-label trial in 73 virally suppressed people with HIV and abdominal obesity found no significant difference in neurocognitive performance between tesamorelin and standard of care at 6 months^[8]. The cognitive question is open.
